The mounting bolt or stud on one side has been repaired by some old wires. Please see the pictures for details. There are lots of little dents and scratches.
I can not find a model number. Some of the bottom reads Cat.FM10 NS2 110 V 50CY 110/120 60CY AMP. 06 NO.11.
The fan does turn on and displays nicely.
The fix with the wire works on the one side.
